You can request an appointment directly with APHT by visiting the Broker Appointment page and completing an appointment request.

Hi there, are you a broker new to Advantage Physicians Healthcare Trust (APHT)? Welcome, we’re excited to bring you aboard! Before you can start selling APHT’s plans, you’ll need to be appointed with APHT. Here's how to get started:

1. Fill out an appointment request
2. Once your request is submitted, our team will review it for approval. Please allow 1-3 business days (excluding weekends and holidays) for our team to review your application.
3. If your application is approved, you will receive a confirmation email. If your application is denied, you will receive an email detailing cause for denial.
4. With approval, we'll create a broker account from which you can start selling policies.

As a reminder, brokers who want to sell APHT’s plans must have an active license with the Louisiana Department of Insurance and hold a producer number in their name.
